I'm on a MacBook Pro (15-inch, 2018) with Mojave (10.14). I'm trying to connect my MacBook to an external monitor (HP Z23N) through HDMI + Apple's usb-c adapter A1621 EMC 2877.

The connection works but the output is extremely blurry.

I know there are a lot of script for fixing this kind of problems. However, many of them do not work anymore and may lead to unexpected results. I'm looking for a fix working fine in that case.

    It looks OK here - though it's quite small. Have you checked what the picture you posted looks like on both your screens? Did you check the monitor is displaying in full resolution? - bearing in mind it's only a 1080p screen, so it's never going to be anything like as crisp as your MacBook's screen. – Tetsujin Oct 4 '18 at 12:06
