The Change Keyboard Type button keeps disappearing, and running the Keyboard Setup Assistant from CLI returns an error No unknown keyboard connected - terminating. Fortunately, installing Karabiner Elements and running it allows the Setup Assistant to run and resolve the problem.

Sep 29, 2016 at 13:09 comment added Tom Gewecke I sounds like your machine thinks the keyboard is ANSI instead of ISO. ANSI keyboards have no key to the left of Z, so < is put at the top left most key. You may need to use Karabiner to fix this.
