Several developers have stepped up to provide CSV data import directly into Health app. Apple has an export function where you can email yourself a zip file, but no import functionality on iOS 8. All that exists is the API apple has that allows third party developers to provide data import functionality on iOS.

If the "old" data which has been collected misses a certain factor which is required by Health, it will be very hard to import this data to Health. However, this is up for the developer. I would personally not develop this feature and spend the time for "upgrades" to an application as it might be a big struggle as explained above.
