After trying to install the 9.3.1 update, which is currently active/signed, I still get the same behavior, even after a restart, so I'm reopening this question. Could something be cached on my device that is causing this?

This is different than the duplicate post because that related to unsigned/pulled updates. My problem still exists after Apple's re-release.

Yep this is a duplicate. I heard about the 9.3 re-release, so assumed it was fixed for ipad air 2. I see from the link on this duplicate post that (as of today, Mar 28 2016), ipad air 2's update is not signed. Thanks all!

Same issue on my end with Dell 2415 monitor.. just putting note here in case others have issue with dell monitor. I've removed Android File Transfer, hoping that'll resolve issue.

I have noticed the same thing, specifically with a Dell 2412 monitor, where my keyboard and mice are connected. Unplugging the USB cable from the rMBP and plugging it back in fixes the issue -- it's fixed when the USB is re-plugged in. (This is with a rMBP'14 running 10.10.5)
